Health Records Requests

Clients may request a copy of their health records by submitting a written request to Amani Counseling Services. Requests can be emailed to AmaniScheduling@gmail.com. Please include your full name, date of birth, contact information, and a description of the records you are requesting.

Requests will be processed in accordance with applicable federal and Texas laws. Additional documentation or identity verification may be required before records are released. If applicable, you will be informed of any fees permitted by law before your records are provided.

Consumer Complaints

If you believe you have been the victim of deceptive business practices or would like to file a consumer complaint, you may contact the Office of the Attorney General of Texas Consumer Protection Division.
